Form can be printed here or it can be mailed to the customer upon request. A void check must be received with signed form. All Payments will be drafted on the 15th of each month with a minimum payment of $20.00 plus a $10.00 convenience fee (one time charge only). All drafts are completed by years end. Monthly charge is payable by bank draft only. Credit cards will not be accepted at this time. Customer must notify TenderCare if there are any changes in bank account, customer contact information etc.EZ Pay Plan is for residential customers only. Must take full program lawn service 1-6. Cut off will be June 1st ( the end of lawn round 2 ). Tree applications can be added to the EZ Pay Plan IF the customer takes full service lawn program. In case of cancellation the remaining balance will be charged on following 15th date.Your bank draft payment plan is continuous and it will continue unless TenderCare is notified in writing 10 days prior to draft. There will be a $30.00 charge for insufficient funds.
